Controlled Drugs

Please go to the website www.cdreporting.co.uk for;

  • Reporting of CD related incidents to the NHS England East of England CDAO. This includes: prescribing errors, dispensing errors, balance discrepancies, lost medication, lost prescriptions and fraudulent attempts to obtain CDs.
  • Authorised Witnesses for CD Destructions, please see below as this has changed May 2024

The new Temporary Authorised Witness module on the Controlled Drug Reporting website (www.cdreporting.co.uk) has now launched.

The way community pharmacies apply for an appropriately authorised person to attend their premises for the purposes of witnessing the destruction of stock schedule 2 CDs in the East of England has now changed.

In most instances, the Controlled Drugs Accountable Officer will no longer arrange for an ‘Authorised Witness’ to attend your pharmacy for the sole purpose of witnessing the destruction of stock schedule 2 CDs.

Instead, any registered healthcare professional (e.g., a registered pharmacy technician or pharmacist) can apply to witness the destruction of stock schedule 2 CDs for a limited period of time in a pharmacy.

Please go to www.cdreporting.co.uk and complete the form ‘Apply to be a Temporary Authorised Witness’ in order to request authorisation to be a witness.

Please be aware that:

  • we will not normally authorise superintendents to act as a witness for the destruction of stock schedule 2 CDs at any pharmacy where they are the superintendent
  • we will not normally authorise sole traders to act as a witness for the destruction of stock schedule 2 CDs at any pharmacy where they are the owner
  • we will not normally authorise individuals to act as witnesses for the destruction of stock schedule 2 CDs at pharmacies that operate as part of a multiple which has 5 or more premises – these multiples will have their own cohort of witnesses
  • Please be aware the information contained within this section does not apply to pharmacies that operate as part of a multiple which has 5 or more premises.

Please get in touch england.ea-cdao@nhs.net if you have any queries, particularly if you are a superintendent or sole trader and would like to discuss potential arrangements to ensure destruction of stock schedule 2 CDs at your pharmacy.
